Initial Visit:  Please bring the following to your scheduled appointment:  

• Physician Prescription
• Your insurance card(s) -

NEW PATIENT FORMS

If you are a new patient, please download the registration, health history and insurance agreement forms, fill them out and bring them to your first appointment. If you don't already have a prescription, please have your doctor fill out our hand therapy referral form. You may download your forms using the links below.
